چکیده

The patient is a 66-year-old woman who had undergone a right extrapleural pneumonectomy 2 years earlier for pleural mesothelioma (stage II) and had received adjuvant chemoand radiotherapy. During the procedure, a Gore-Tex patch was used to reconstruct the diaphragm. Six months after surgery, a follow-up MRI was ordered due to high transaminase levels, and herniation of the liver into the thorax was observed (Fig. 1a). CT showed the patch situated under the wall (Fig. 1b, arrow). In spite of this complication, the patient presents good quality of life and has required no treatment. Extension of liver tissue into the thorax following a right extrapleural pneumonectomy for malignant pleural mesothelioma.

A 56-year-old male underwent a right extrapleural pneumonectomy for malignant pleural mesothelioma. Two years after the operation, an intrathoracic mass was suspected of recurrence by imaging, however, biopsy revealed that the mass was actively proliferating liver tissues. We discussed this condition in terms of ectopic liver proliferation.

Post-pleuropneumonectomy herniation of liver mimicking major pulmonary embolism.

Following right-sided pneumonectomy and hemidiaphragm resection in a 58-year-old man with epithelioid mesothelioma, acute respiratory insufficiency and life-threatening circulatory collapse developed after a forced Valsalva maneuver. Major pulmonary embolism was diagnosed on clinical grounds, however computed tomography revealed herniation of the liver into the right hemithorax.

We report a case of a 58-year-old Japanese man with epithelioid-type MPM, with a left chest wall protruding tumor, who underwent extrapleural pneumonectomy and chest wall resection. Postsurgical pathology showed that surgical margins were free of mesothelioma cells. Positron emission tomography taken 5 years after surgery showed no recurrence at any site, suggesting that long-term survival coul...
